PCOM Healthcare Centers
Our multidisciplinary team provides comprehensive care for older adults, with physicians who are board-certified in geriatrics, palliative care and pain management. We care for older adults through every stage of their healthcare needs, providing care in our offices, skilled nursing facilities and hospitals. Our goal is to help each patient manage their health, maintain independence and enjoy the best possible quality of life through care tailored to their individual needs.
Our geriatric services include preventive and primary care, management of multiple medical conditions and medications, pain and symptom management, and support for patients and families navigating changes in health and function. For patients experiencing memory loss or changes in thinking, specialized evaluation and care are also available through PCOM’s Memory Center.
