The Memory Center at PCOM combines the expertise of neuroscientists with the clinical disciplines of geriatric medicine, clinical psychology and gerontological nursing to provide diagnosis and treatment for cognitive decline. This interdisciplinary approach enables accurate diagnosis, patient-centered treatment planning, and proactive advance care strategies to improve outcomes for patients and their families.
Clients at the Memory Center are thoroughly evaluated by board-certified geriatricians and doctoral-level psychologists working together to reach diagnostic consensus. The diagnostic journey is overseen by skilled care coordinators who provide support, educational resources and assistance in patient-centered treatment decision-making. There are currently close to 7 million people in the U.S. living with Alzheimer Disease; that number is projected to grow as the population ages. The PCOM Memory Center enables early identification of individuals with cognitive decline so that accurate diagnosis and appropriate next steps in treatment are initiated. In addition to benefiting individuals with cognitive decline by possibly delaying and preventing progression to dementia, the PCOM Memory Center will establish a data bank for research and development of broad programs that can be scaled for primary healthcare practice.
Through community engagement, advocacy and affiliation, education and research, we endeavor to improve the understanding and management of cognitive health and improve the quality of care for individuals with memory impairment and their families and caregivers.
